Admissions to Crouse Community Center are based on medical necessity. Criteria for admissions will be determined based on an admission screening to determine the level of care that each resident requires.

An application for admission, along with a medical statement completed by a physician, need to be filled out and submitted to the Social Services Director. An application can be requested by calling the Social Service Office, by downloading the pdf version under the "Related Links," or by e-mail request at nancy@crousecommunity.com.

Crouse Community Center accepts Medicare, Medicaid and Private Pay residents. Questions about other payors sources may be directed to the Social Service Office or the Business Office at 315-684-9595.
