To apply for admission to Crouse Community Center you must complete application forms, which consist of the following:
A personal application form to be completed by the person or family.
A medical application to be completed by the physician.
In addition, New York State requires a PRI (Patient Review Instrument) and Screen be completed before anyone can enter a nursing home. The PRI and Screen can be completed by the Public Health Nurse if the applicant is at home, OR by the hospital staff if the applicant is hospitalized.
